Community Nursing Overview
EqualRise Healthcare provides community nursing and health support services to assist participants with health-related tasks that require trained or qualified staff. All nursing support is delivered within appropriate scope, registration, and documented care plans.
Our workers are trained to recognise when escalation to a GP, nurse, pharmacist, or emergency services is required — and to take appropriate action promptly.

What We Provide
Medication Support & Prompting
Assisting participants with medication prompting, administration (within scope), and monitoring for side effects or adverse reactions.
Health Monitoring
Within-scope health monitoring including vital signs, general observations, and reporting changes in health status.
Care Plan Implementation
Supporting the implementation of documented care plans developed by registered nurses, GPs, or other health professionals.
Escalation & Liaison
Recognising when to escalate concerns and liaising with GPs, nursing staff, pharmacists, or emergency services as required.
