What is Support Coordination?

Support Coordination is an NDIS-funded service that helps participants understand, implement, and get the most from their NDIS plan. A Support Coordinator works with you to connect with the services and supports that best meet your goals.

Support Coordination builds your capacity to exercise choice and control, navigate the NDIS system, and manage your own supports over time.

Who May Benefit?
Anyone with NDIS funding for Support Coordination — particularly those with complex support needs, multiple providers, or who are new to the NDIS.

Levels of Support Coordination

Level 2 – Support Coordination
Assistance to strengthen your ability to connect with and coordinate your supports, implement your plan, and resolve day-to-day issues.
Specialist Support Coordination
High-intensity coordination for people with complex support needs, including complex situations, risk, or multiple system involvement. Delivered by a qualified specialist.
